Procedures in referring patient to Tawam Hospital

Requirements:

• A referral letter from patient’s current treating physician.
• A valid passport copy.
• Non National patients are required to have a current resident’s visa.
• Daman or Thiqa Insurance coverage.
• Other insurance coverage is accepted but patient needs to bring a confirmation letter from the insurance company stating that the company is willing to pay for the cost of treatment of the patient.

• If the patient does not have insurance coverage, he/she will be considered as a self paying patient, and will be billed for all consultations and treatment.

Procedures:

• The Referral letter and medical report should be faxed to the Deputy Medical Director’s office or to the Office of the Oncology Department for acceptance.

• Following submission of the referral or medical report, an acceptance letter from the Tawam Deputy Medical Director’s Office will be sent to the referring hospital or to the referring doctor or directly to the patient. Following that, an outpatient appointment or date of admission will be relayed to you by telephone
• Other requirements (points 2 to 6) can be submitted when the patient comes for opening Tawam file.
